Enroll at JC Sunshine Childcare

We are excited to welcome your family to JC Sunshine Childcare. Please complete the steps below before your child’s first day.

Step 1: Complete Enrollment Packet

Step 2: Complete Medical Consent Form, if needed

New Child Medical Form

Step 3: Upload Required Documents

This can either be inside your Jotform or a separate form later.

Documents parents may need:

Immunization record
Medical statement/physical
ACS/voucher paperwork, if applicable
IEP/IFSP, if applicable
Custody/court documents, if applicable

Enrollment is not complete until all required forms, documents, and payments are received and confirmed by JC Sunshine Childcare.

Step 4: Pay Registration Fee / First Week Tuition/ Submit Voucher for signing

Step 5: Pay Weekly Tuition

Payment Policy

Tuition is due every Monday by 3:00 PM. Tuition holds your child’s childcare space and is not based only on attendance. Late payments may result in late fees, pause of care, or termination of care. ACS families are responsible for any parent fee, co-pay, private balance, or amount not covered by the voucher.
